Full-Time | Monday–Friday | 8:00 AM – 5:00 PM

We are seeking a dedicated Case Manager/Therapist to support transitional-age youth (18–24) experiencing homelessness. This role blends case management, therapeutic support, life skills development, and housing stabilization in a structured, mission-driven environment.

What You’ll Do

• Conduct comprehensive intakes and clinical assessments
• Develop and implement individualized service plans
• Provide individual counseling and facilitate psychoeducational/life skills groups
• Connect youth to housing, employment, education, and mental health resources
• Provide crisis intervention and structured daily support
• Maintain compliant documentation (HMIS and internal systems)
• Collaborate with a multidisciplinary team to support successful transitions

What You Bring

Required:

• Active California licensure as one of the following:
• LMFT, LPCC, LCSW, or Licensed Psychologist (State of California)
• Spanish bilingual
• 
  

Preferred:

• Experience working with at-risk youth or individuals experiencing homelessness
• Strong documentation and organizational skills
• Trauma-informed, strength-based approach
• Ability to build trust and rapport with young adults
