Pay: $110,000.00 - $140,000.00 per year


Job description / Position Overview


We are seeking an APP who is looking for something different than the traditional hospital or corporate healthcare environment. This role offers a high degree of clinical autonomy, flexibility, and independence. As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and provide care in a way that prioritizes outcomes. This is an ideal role for a provider who is confident in their clinical decision-making, values patient relationships, and prefers a more flexible and self-directed work environment.

  • Autonomy in Clinical Practice - You will evaluate, diagnose, and treat patients independently, with support from experienced wound care leadership when needed.

  • Mobile Care Delivery - Care is provided in the home and facility setting, eliminating many of the barriers patients face when accessing traditional wound clinics.


Care Coordination

  • Collaborate with home health agencies, skilled nursing facilities, and referring providers
  • Communicate treatment plans and updates to the broader care team
  • Support transitions of care following hospital or SNF discharge
  • Educate patients and caregivers on wound management and prevention


Documentation & Compliance

  • Maintain accurate, timely, and compliant documentation
  • Ensure documentation meets Medicare and payer requirements
  • Follow HWMS clinical protocols and workflows
  • Participate in periodic chart reviews and quality initiatives


Qualifications Required

  • Active, unrestricted North Carolina license
  • Board Certification (FNP, AGNP, PA-C, or similar)
  • DEA license (or ability to obtain)
  • Valid driver’s license and reliable transportation


Preferred

  • Wound care experience or certification (CWOCN, CWON, WCC, or similar)
  • Experience in home health, SNF, or post-acute care settings
  • Familiarity with Medicare documentation and billing practices


Compensation & Benefits
