Job Title: Primary Care Physician

Location: North Hollywood, California 91601

Job Type: Full-time

Work Setting: PACE center and community-based senior care

Schedule: Monday through Friday, 8:00 AM to 5:00 PM, including home visits, with rotating phone-only call a few nights or weekends per month

Pay: $275,246 - $330,595 base salary, plus quarterly bonus up to 10% of salary

This is an opportunity to build deeper patient relationships, work with a collaborative interdisciplinary team, and make a meaningful impact on a vulnerable senior population.

With an average of about 8 patients per day and no hospital rounds, this role offers the ability to focus on thoughtful, relationship-centered care rather than high-volume throughput.

 

Responsibilities

Provide comprehensive primary care for older adult participants in a PACE and community-based setting

Perform in-person assessments and develop individualized care plans in collaboration with the interdisciplinary team

Partner closely with advanced practice providers and other clinical team members to support coordinated care

Coordinate discharges and transitions across acute, short-term, and long-term care settings

Complete home visits as needed to support continuity and quality of care

Participate in rotating phone-only call coverage a few nights or weekends per month

 

Required Qualifications

MD or DO from an accredited program

Successful completion of an ACGME-accredited residency program

Active physician license without restrictions in the applicable state

State CDS certification and DEA registration

Eligible for enrollment to provide services to Medicare and Medi-Cal participants

Ability to obtain and maintain CPR and First Aid certification within 180 days of hire

Experience working in an interdisciplinary team structure

 

Preferred Qualifications

Board certification preferred

2-5 years of primary care experience, ideally with geriatric populations

Experience in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics

Experience in value-based care, senior care, or PACE settings is a plus

Experience collaborating effectively with advanced practice providers is preferred

Work Environment

This role combines center-based and community-based care with a collaborative team model focused on whole-person support for older adults. Physicians work regular weekday hours with limited rotating phone-only call and no hospital rounds.
